Athletics – Sports Physicals
Your child may participate in the Longfellow Middle School. Longfellow competes in the Three Rivers Conference which is composed of teams from Aquinas, GET, Holmen, LaCrescent, Logan, Lincoln, Longfellow, Onalaska, Sparta, Tomah, Viroqua, West Salem, Westby, and Winona Middle Schools.
Prior to participating in any practice or competition session, a student must turn in a Wisconsin Interscholastic Athletic Association (WIAA) athletic physical card (Click on FORMS link to the left). If your child had a physical examination after April 1, 2020 it will be good for the entire school 2021-22 school year.
It is suggested that you make an appointment for a physical examination as soon as possible during the summer. A link to the physical form is below. Hard copies are also available in the Longfellow office.
